Attendee Photo Consent Release Form

This year ASA will require consent forms for every attendee. The purpose of this is to allow ASA to utilize photographs, videos, and other forms of recording and share these images and recordings of conference highlights on our website, social media, and other outlets.


The form is at the end of the program. There are multiple options for submitting the form. You may complete the form and submit prior to the conference by scanning and emailing to asa@marshall.edu with the subject line “ASA 2019 consent form;” you may bring the completed copy to registration when you get your conference materials; or you may complete one at the registration desk onsite and turn it in when you get your materials.


If you are under the age of 18 years of age at the time of the conference, you will need to contact ASA office manager, Ann E. Bryant, at mullins88@marshall.edu prior to the conference.


If you do not wish to have photos, video, or other recordings taken of you during the conference, please contact ASA office manager, Ann E. Bryant, at mullins88@marshall.edu.
